Thrive to Five Program Handbook
The Program Handbook gives you information about our Family Resource Center, including location, contact information, procedures, and more! The Handbook also contains three documents we ask all families to complete and return to participate in future classes and events - Parent Information Form, Photo and Video Consent Form, Family Handbook Acknowledgment Form.

Classes
Registration Process
Thrive to Five uses a “lottery” style process to select families for class registration in all classes. This will provide an opportunity for all families to attend classes and activities. Due to our activities often reaching maximum participants allotted, you may not be chosen to participate in every class each month. After you have added your name to an Interest List, you will receive an automated confirmation email to inform you that we have received your submission. If you are selected to participate in a class, you will receive an invite via text message/email at least 7 days prior to the start of the class, to inform you that you have been selected. Please note that you are not registered for an activity until you have received the class invite and confirmation email or text from a staff member at the Thrive to Five Family Resource Center. If you have any questions, please do not hesitate to contact Thrive to Five at 480-682-8415.

Do you need information about services and resources for you and your 0 to 5 year old child? Fill out our Referral Request Form and a Thrive to Five staff member will contact you within 3-5 business days to discuss your needs and connect you to local organizations.
